Job Summary
The Outpatient Dialysis RN is responsible for delivering safe, high-quality hemodialysis care in an outpatient setting. The nurse evaluates patients, collaborates with physicians, and provides patient-centered treatment aligned with professional nursing standards.
Key Responsibilities
Evaluate patient conditions and assist physicians with care plans
Plan, implement, and document nursing care based on patient needs
Identify patient priorities and adjust care accordingly
Develop and maintain individualized dialysis care plans
Initiate assessments based on patient conditions and provider orders
Perform therapeutic nursing interventions
Manage assigned caseload of dialysis patients
Document care thoroughly based on practice standards and acuity
Support evidence-based practice and continuous improvement
Provide patient-centric education and support
Perform other related duties as assigned
Required Skills & Experience
Minimum 1 year experience in an outpatient dialysis center
Minimum 1 year ICH dialysis experience
Experience with Fresenius T-Machine and portable RO systems
Ability to manage:
3 patients for put-on/take-off
9 patients with 2 PCTs for medication administration and assessments
Preferred Skills & Experience
Telemetry experience
Chronic hemodialysis experience
Strong dialysis documentation skills (care plans, monthly nursing notes, foot checks, med review)
Education
Required: ASN
Preferred: BSN
