Position Summary
A healthcare facility in Hyannis, Massachusetts is seeking a qualified Licensed Clinical Social Worker or Behavioral Health Clinician to provide comprehensive psychosocial assessments, treatment planning, and care coordination for patients requiring behavioral health services.
The clinician will work collaboratively with psychiatrists and multidisciplinary teams to ensure effective treatment planning, crisis intervention, discharge planning, and coordination of community-based services to support patient recovery and continuity of care.
Responsibilities
Conduct initial clinical assessments for patients presenting with acute behavioral health needs in accordance with state mental health requirements
Develop initial treatment plans based on assessment findings and communicate plans to the multidisciplinary care team
Lead treatment planning and discharge coordination in collaboration with psychiatrists and healthcare team members
Facilitate individual therapy, family counseling, and group therapy sessions
Support patient care planning to ensure ongoing treatment in a safe and therapeutic environment
Evaluate medical and psychosocial needs to coordinate appropriate home care and community support services
Link patients with community programs and support services to promote long-term recovery and stability
Assist patients and families in obtaining medical equipment or supportive services required for safe home functioning
Guide patients and families through decision-making processes related to placement or facility transfer when necessary
Serve as a liaison between patients, families, and community agencies
Provide crisis prevention and intervention services when needed, particularly during discharge transitions
Provide coverage and support for inpatient and partial hospital behavioral health programs
Communicate with managed care organizations to obtain authorization for required levels of care
Maintain compliance with regulatory standards and documentation requirements
Participate in community outreach and education initiatives
Provide compassionate, patient-centered service to patients, families, and healthcare team members
Required Qualifications
Ability to read, write, and communicate effectively in English
Active Massachusetts license in one of the following:
Licensed Independent Clinical Social Worker (LICSW)
Licensed Clinical Social Worker (LCSW)
Licensed Mental Health Counselor (LMHC)
Licensed Marriage and Family Therapist (LMFT)
Doctorate in Psychology (PhD)
Crisis Prevention Intervention (CPI) or Management of Aggressive Behavior (MOAB) Certification required
Basic Life Support (BLS) certification through the American Heart Association
