Educate patients and families on disease progression, symptom control, and the end-of-life process
Provide emotional and psychosocial support to patients and their loved ones
Collaborate with physicians, social workers, chaplains, and other team members
Accurately document care in electronic medical records (EMR)
Participate in interdisciplinary team meetings and case conferences
Maintain compliance with all state, federal, and hospice care regulations
Be available for on-call responsibilities, if required
Qualifications:
Active and unrestricted RN license in Massachusetts
1–2 years of experience in hospice or palliative care preferred
Strong clinical assessment and critical thinking skills
Excellent communication, interpersonal, and organizational skills
Compassionate, empathetic, and capable of working independently
Proficient in using Electronic Medical Records (EMR)
Valid driver’s license and reliable transportation (if community/home visits are involved)
