Certified Home Care experience required.
OASIS and EPIC familiarity preferred.
Every other weekend required.

Job Title: Home Care Registered Nurse - Rotational Shift
Job Summary: The Home Care Registered Nurse (RN) is responsible for providing professional nursing care to patients in their homes. This includes assessment, care plan development, implementation, and evaluation of home nursing care on an individual and continuous basis. The RN ensures compliance with organizational policies, procedures, and external regulations while delivering high-quality patient-centered care.
Key Responsibilities:
Ensure compliance with physician orders and maintain appropriate visit frequency.
Document patient care accurately, reflecting progress toward discharge goals.
Perform comprehensive nursing assessments and medication reconciliation.
Develop, update, and implement individualized care plans based on patient status.
Provide patient education and document teaching strategies in medical records.
Utilize interdisciplinary teams and ensure proper documentation of care activities.
Identify and comply with payor guidelines and authorization requirements.
Act as a liaison between physicians, managers, and healthcare teams for effective coordination of services.
Supervise home health aides, conduct required supervisory visits, and participate in performance evaluations.
Maintain awareness of updated healthcare standards and integrate best practices into nursing care.
Meet productivity requirements and complete assignments independently and in a timely manner.
Other duties and projects as assigned.
Required Skills & Experience:
Minimum of 2 years of relevant nursing experience.
Experience in an acute care or hospital setting.
Strong adaptability to changing patient needs and a commitment to patient-focused care.
Ability to work collaboratively in a team and demonstrate accountability for patient outcomes.
