Job Overview:

Seeking a compassionate and skilled Licensed Practical Nurse (LPN) to provide high-quality care in a long-term care facility. The LPN will support daily clinical operations under the direction of an RN or physician, ensuring resident comfort and regulatory compliance.

Key Responsibilities:

• Deliver direct nursing care, including medication administration and vital sign monitoring

• Document care accurately and update resident care plans

• Observe and report changes in patient condition

• Promote emotional and physical well-being of residents

• Supervise and support CNAs and care staff

• Adhere to infection control and long-term care standards

Requirements:

• Active LPN license in New York

• Graduate of an accredited Practical Nursing program

• BLS certification

• Experience in long-term or geriatric care preferred

• Strong communication, observation, and teamwork skills
