Position Overview
We are seeking an experienced RN House Supervisor to provide leadership and clinical oversight during off-shifts (evenings, nights, weekends, holidays) for a 180-bed long-term care facility. The House Supervisor serves as the clinical and operational resource for five specialized units, ensuring safe patient care, regulatory compliance, staff mentorship, and effective patient flow across the organization.
This role requires strong charge experience, crisis management skills, and the ability to lead in a high-responsibility environment.

Key Responsibilities
Provide clinical leadership and supervision across all facility units during assigned shifts.
Serve as a resource for problem-solving and clinical decision-making.
Facilitate staff mentoring, coaching, and performance oversight.
Support centralized staffing and productivity standards.
Oversee new admissions and patient flow.
Manage emergency preparedness, code response, and escalation processes.
Ensure compliance with regulatory standards (DPH, TJC, OSHA, HIPAA).
Monitor and report sentinel events and incidents.
Handle de-escalation and management of assaultive behaviors.
Support documentation processes including death reporting and regulatory compliance.
Maintain infection control and isolation protocol standards.

Required Qualifications
Active New York State RN License.
Minimum 2 years of experience.
Previous Charge Nurse experience – Required.
Experience in Skilled Nursing Facility (SNF) setting.
EPIC EMR experience – Required.
BLS Certification.
Strong knowledge of federal and state regulations related to long-term care.
