Schedule

  • Shift: Day Shift (5x8 Hours)
  • Timing: 8:00 AM – 4:30 PM

Compensation

  • Travelers: $62 – $66/hour ($2,480 – $2,640/week, based on experience)
  • Local Candidates: $48 – $52/hour ($1,800 – $1,950/week, based on experience)

Position Overview

A reputable healthcare facility is seeking an experienced RN Case Manager to support care coordination and discharge planning in a fast-paced acute care hospital environment. The ideal candidate will be highly organized, able to work independently, and experienced in managing complex patient care transitions.


Key Responsibilities

  • Coordinate patient care and discharge planning in an acute hospital setting
  • Collaborate with interdisciplinary teams including IV Therapy, Physical Therapy, Respiratory Services, Radiology, Social Services, and more
  • Participate in treatment plan development and patient care planning
  • Organize and lead family meetings regarding patient care and discharge plans
  • Arrange post-acute services including DME, Skilled Nursing Facility (SNF) placement, and ambulance transportation
  • Work with HMOs, IPAs, and other payer systems to ensure appropriate care coordination
  • Ensure smooth transitions of care and timely discharge processes
  • Maintain accurate documentation and communicate effectively with care teams

Required Qualifications

  • Active Registered Nurse (RN) License (CA)
  • Minimum 2 years of acute hospital Case Management experience
  • Proven experience in care coordination and discharge planning in a fast-paced environment
  • Strong knowledge of healthcare systems including HMOs, IPAs, and post-acute care services
  • Ability to work independently and manage multiple priorities effectively
