Job Title: Registered Nurse Hospice
Location: Barre, VT
Duration: 16 Weeks
Schedule: Day Shift | Monday – Friday | Some Weekend Coverage

Pay Rate: $2500 – $2700/week (Depending on Experience)

Position Overview

"Navitas Healthcare, LLC" is seeking a compassionate and experienced Hospice Registered Nurse (RN) to provide high-quality end-of-life care in patients' homes. This role focuses on symptom management, patient advocacy, family education, and coordinated hospice care. The ideal candidate will have strong Home Health, OASIS, and Case Management experience and be comfortable working independently while collaborating with interdisciplinary care teams.

Key Responsibilities

Hospice Patient Care

• Deliver individualized hospice and end-of-life nursing care.
• Assess patient needs and monitor changing conditions.
• Support patients and families throughout the hospice journey.
• Promote dignity, comfort, and quality of life.

Pain & Symptom Management

• Monitor and manage patient symptoms.
• Administer and evaluate treatments and interventions.
• Provide comfort-focused nursing care.
• Educate caregivers on symptom management and care plans.

Case Management

• Coordinate patient care plans and services.
• Manage assigned patient caseloads.
• Complete OASIS documentation accurately and timely.
• Communicate effectively with providers and interdisciplinary teams.

Team Collaboration

• Collaborate with hospice interdisciplinary team members.
• Coordinate care with physicians and community healthcare providers.
• Support family caregivers through education and guidance.

Documentation

• Complete required charting and clinical documentation.
• Utilize Homecare Homebase (HCHB) EMR.
• Maintain compliance with OASIS and quality standards.

Required Qualifications

Licensure

• Active Vermont RN License.

Certifications

• BLS/BCLS Certification Required.

Experience

• Minimum 1 year of Home Health experience.
• OASIS Documentation experience required.
• Case Management experience required.
• Hospice experience strongly preferred.

EMR

• Homecare Homebase (HCHB) experience required.
