• Hours: 7:00 AM – 7:30 PM

Compensation:

• Traveler: $50-$53/hr
  
• Local: $40/hr

Position Overview

We are seeking a dedicated healthcare professional to provide high-quality patient care within a Skilled Nursing Facility (SNF) setting. The ideal candidate will possess strong clinical skills, excellent documentation practices, and a commitment to delivering patient-centered care while collaborating with interdisciplinary healthcare teams.

This position offers an opportunity to work in a supportive clinical environment focused on improving patient outcomes and maintaining the highest standards of care.

Responsibilities

• Provide direct patient care in accordance with facility policies, procedures, and standards of practice.
• Monitor, assess, and document patient conditions and responses to treatment.
• Administer medications and treatments as prescribed.
• Maintain accurate, complete, and timely patient records and documentation.
• Collaborate with physicians, nurses, therapists, and other healthcare professionals to coordinate patient care.
• Ensure compliance with infection prevention, patient safety, and regulatory standards.
• Educate patients and family members regarding treatment plans and care needs.
• Respond promptly and appropriately to changes in patient conditions.
• Participate in care planning and interdisciplinary team meetings.
• Perform additional duties as assigned to support patient care and facility operations.

Required Qualifications

Licensure & Certifications

• Active applicable State License required.
• Current BLS Certification required.

Experience

• Skilled Nursing Facility (SNF) experience preferred.
