Physical Therapist – Acute Care

Location: Apple Valley, California

Contract Duration: 13 Weeks

Schedule

  • 5x8 Hour Shifts
  • Tuesday – Saturday
  • 9:00 AM – 5:30 PM

Position Overview

We are seeking a skilled Acute Care Physical Therapist to provide evaluation, treatment, and rehabilitation services to patients across a variety of acute care settings. The ideal candidate will have experience working with medically complex patients and developing individualized treatment plans to maximize functional mobility and independence.

Required Qualifications

  • Active California Physical Therapist License
  • Minimum 1 year of Acute Care Physical Therapy experience
  • BLS Certification
  • First-time travelers accepted

Key Responsibilities

  • Evaluate patients and develop individualized treatment plans
  • Establish measurable goals and monitor patient progress
  • Perform reassessments and modify treatment plans as needed
  • Provide patient and family education regarding rehabilitation and home exercise programs
  • Coordinate discharge planning and continuity of care
  • Document evaluations, treatments, progress notes, and care plans accurately
  • Collaborate with physicians, nurses, case managers, and interdisciplinary teams
