Nurse Supervisor – Care Management / Transitional Care

Schedule: 40 Hours per Week | 8 Hours per Day | 5 Days per Week
Start Date: July 6, 2026
Pay Rate: $100 - $110/hour
We are seeking an experienced Nurse Supervisor to lead the Care Management and Transitional Care team. This role will oversee care coordination initiatives, discharge planning, utilization review activities, and readmission reduction programs while supporting safe patient transitions from hospital to community settings.
Bachelor's Degree in Nursing required.
Active RN License with NYSED Verification.
BLS Certification required.
CCM (Certified Case Manager) Certification required.
PRI Certification required.
Minimum 7 years of nursing experience.
Minimum 5 years of supervisory or leadership experience.
Minimum 2 years of experience in Care Management, Transitional Care, Discharge Planning, or Utilization Review.
Strong leadership, communication, and performance management skills.
Supervise Care Managers, Care Coordinators, and support staff.
Oversee discharge planning and transitional care services.
Support initiatives focused on reducing hospital readmissions.
Monitor patient outcomes, staff performance, and quality improvement activities.
Develop and implement departmental policies and procedures.
Collaborate with interdisciplinary teams to improve patient care transitions.
Assist with staff education, compliance, and performance improvement programs.
Master's Degree in Nursing.
Leadership experience within Care Management, Transitional Care, or Population Health programs.
