Title: RN – Adult Care / Home Care
Location: New York, NY
Duration: 13 weeks
Shift: 9:00 AM-5:00 PM

Job Responsibilities

• Assess, plan, implement, and evaluate patient care plans in collaboration with healthcare professionals.
• Provide direct nursing care in patients' homes.
• Administer medications and monitor patient responses.
• Perform wound care, dressing changes, catheter care, and IV therapy.
• Monitor patient vital signs and report adverse reactions promptly.
• Educate patients and families regarding treatment plans and ongoing care.
• Collaborate with interdisciplinary teams on discharge planning and continuity of care.
• Maintain accurate patient records and clinical documentation.
• Adhere to established nursing standards, best practices, and protocols.

Required Experience

• Minimum 2 years of Med/Surg experience in an acute care hospital or post-acute setting.
• Most recent experience must be within the past year.
• Previous Home Care experience (traveling to patient homes and providing care).
• OASIS documentation experience.

Education Requirements

• Associate Degree in Nursing (ADN/ASN) from an accredited nursing program

Required Certifications & Licensure

• Active New York State RN License
• Primary Source Verification of RN License
• AHA Basic Life Support (BLS)

Additional Notes

• Candidates must have recent Home Care experience.
• Experience completing OASIS assessments is required.
• This position involves traveling to patient homes throughout the assigned service area.
