Job Title: Home Health Speech-Language Pathologist (SLP)
Location: Mount Angel, OR & Surrounding Areas
Start Date: ASAP
Shift: 3x8 Hour Days (Wednesday–Friday, 8:30 AM – 5:00 PM)
Schedule Note: 4x10s or 3x12s schedules are not available


Role Overview

We are seeking a dedicated Home Health Speech-Language Pathologist (SLP) to provide one-on-one patient care in a home health setting. This role focuses on delivering high-quality speech therapy services, managing patient caseloads efficiently, and ensuring accurate documentation using standardized systems.


Key Responsibilities

  • Provide speech-language therapy services to patients in home health settings
  • Manage a caseload of up to 6 patients per day (productivity standard: 30 points)
  • Perform patient assessments, develop care plans, and track progress
  • Complete accurate and timely documentation using required systems
  • Travel to patient homes and healthcare facilities within the assigned region
  • Collaborate with interdisciplinary care teams to ensure optimal patient outcomes
  • Maintain compliance with healthcare regulations and organizational standards

Required Qualifications

  • Minimum 1 year of skilled home health experience
  • Active SLP licensure (or pending license accepted)
  • BLS (Basic Life Support) certification required
  • Experience with OASIS documentation (required)
  • Valid driver’s license and reliable transportation
  • COVID-19 vaccination (primary series + one booster required)

Preferred Qualifications

  • Experience with EPIC charting system
  • Strong organizational and time-management skills
  • Ability to work independently in a home health environment

Work Environment & Additional Details

  • Home Health setting
  • Business casual attire or scrubs permitted
  • No weekend or on-call requirements
  • No floating between units
  • Block scheduling not available
  • Local candidates accepted (within 50–75 miles)

Travel & Coverage

  • Travel required within assigned territory (typically optimized for minimal drive time)
  • Occasional travel radius may extend up to 50+ miles
  • Mileage reimbursement provided at standard rate

  • First-time travelers are welcome
  • Candidates must be comfortable working independently and managing schedules efficiently
