Job Title: Licensed Clinical Social Worker (LCSW)

Location: Seattle, WA 98122

Schedule: Day Shift – 5x8 Hours (08:00 AM – 04:30 PM)

Duration: 13 Weeks

Compensation:

• FROM $1,800 per week (Local Candidates)
• FROM $2,200 per week (Traveler Candidates)

Key Responsibilities:

• Conduct psychosocial assessments and develop care plans for acute care patients.
• Coordinate discharge planning, including arranging community resources and post-acute care services.
• Provide crisis intervention, case management, and collaborate with interdisciplinary teams.
• Assist with advance care planning, protective services, and patient advocacy.
• Maintain accurate and comprehensive documentation.
• Ensure continuity of care through referrals and appropriate resource linkage.

Required Licenses & Certifications:

• Active state licensure (LSWAIC or equivalent)
• BLS certification (AHA)

Education & Experience:

• Master’s Degree in Social Work or related field (required)
• Minimum 1 year of recent inpatient case management experience
