Job Title: LPN / LVN – Long Term Care (LTC)

Location: Wayland, NY

Schedule:
10:00 PM – 8:00 AM (Wednesday – Friday)
Sunday – Tuesday Off

Contract Duration: 13–52 Weeks Available

Position Overview:

"Navitas Healthcare, LLC" is seeking an experienced Licensed Practical Nurse (LPN/LVN) to provide care for individuals with developmental disabilities in a long-term care setting. The LPN will work under the supervision of a Registered Nurse and assist with medication administration, treatments, patient monitoring, and daily care activities.

Key Responsibilities:

  • Administer medications and injections as prescribed

  • Perform tube feedings and hang IVs as required

  • Collect blood samples (venipuncture certification preferred)

  • Provide colostomy care and wound care

  • Monitor patient conditions and report changes to nursing leadership

  • Take and record vital signs including temperature, pulse, respiration, and blood pressure

  • Collect specimens for laboratory analysis

  • Apply dressings and perform basic nursing procedures

  • Assist with bathing, feeding, lifting, and transferring residents

  • Maintain resident living areas and assist with daily care needs

  • Complete accurate documentation and patient records

  • Float to other homes/facilities as needed based on staffing requirements

Requirements:

  • Active New York State LPN License

  • Minimum one year of nursing experience after licensure

  • SCR and SEL clearance required prior to start (may take 2–4 weeks)

  • Experience in LTC, developmental disabilities, or residential care preferred

  • Strong communication and patient care skills

  • Ability to work independently and as part of a healthcare team

Additional Information:

  • This is a mandated facility.

  • If staffing shortages occur, nurses may be required to remain on duty until relief arrives.

  • Floating to other homes may be required based on operational needs.
