Job Title: Master of Social Work (MSW) – Travel Contract
Location: Napa, CA
Contract Length: 13 Weeks
Shift: Day Shift | 5x8s | 8:00 AM – 4:30 PM
We are seeking a compassionate and experienced Master of Social Work (MSW) professional for a travel assignment in Napa, California. The ideal candidate will provide psychosocial support, discharge planning, care coordination, and advocacy services while collaborating closely with interdisciplinary healthcare teams.
Responsibilities
Conduct comprehensive psychosocial assessments of patients and families
Develop and implement individualized care and discharge plans
Provide crisis intervention, counseling, and emotional support services
Assist patients with accessing community resources, financial assistance programs, and support services
Collaborate with physicians, nurses, case managers, and healthcare professionals to coordinate patient care
Advocate for patients’ rights and promote patient-centered care
Facilitate discharge planning to ensure safe and effective transitions of care
Identify barriers to treatment and develop strategies to address patient needs
Maintain accurate and timely documentation in accordance with facility and regulatory standards
Participate in multidisciplinary team meetings and case reviews
Educate patients and families regarding available healthcare and community resources
Ensure compliance with hospital policies, state regulations, and ethical standards of social work practice
Requirements
Master’s Degree in Social Work (MSW) from an accredited program required
Current California Social Work licensure or eligibility required
Previous experience in a healthcare, hospital, or acute care setting preferred
Strong knowledge of discharge planning, care coordination, and community resource management
Excellent communication, interpersonal, and counseling skills
Ability to work effectively in a multidisciplinary healthcare environment
Strong assessment, critical thinking, and problem-solving abilities
Experience in acute care, medical social work, or case management preferred
Familiarity with EMR systems preferred
Knowledge of Medicare, Medicaid, and healthcare regulations preferred
Experience working with diverse patient populations preferred
