RN Case Manager [PERMANENT ROLE] - RNCM 0529 IS#02

Job Title: RN Case Manager
Location: Fort Walton Beach, FL 32547
Shift: Days (Rotating Weekends) – 8:00 AM to 4:30 PM
Position Type: Full-time / Permanent

Compensation & Benefits: $34.25 – $47.10/hr (based on experience)

Job Summary:

• Coordinate patient care and support patients/families throughout treatment
• Develop individualized care and discharge plans
• Ensure safe and effective transitions of care
• Collaborate with physicians and interdisciplinary teams
• Maintain compliance with hospital and payer guidelines

Key Responsibilities:

• Guide patients and families on care plans, insurance, and discharge process
• Conduct psychosocial and clinical assessments
• Develop and implement care and discharge plans
• Coordinate patient education and family meetings
• Collaborate with physicians, rehab teams, and staff
• Ensure timely and safe discharge planning
• Document patient needs and communicate across teams
• Advocate for appropriate patient care decisions

Required Qualifications:

• Active RN license
• Minimum 2 years recent acute care experience (within last 3 years)
• Experience in ICU, CCU, Med-Surg, LTAC, or Case Management
• Associate Degree in Nursing or Diploma
