Job Title:  Registered Nurse - Home Health
Location: St. Johnsbury, VT
Type: Contract | 13 Weeks
Shift & Schedule: Days | Monday–Friday | Possible Weekend Rotation | Guaranteed 36 Hours/Week

Position Overview:
Navitas Healthcare, LLC is seeking Home Health Registered Nurse (RN) for an exciting Travel or Local job in St. Johnsbury, VT.

Responsibilities

• Perform patient admissions, evaluations, and care plan development
• Deliver skilled nursing care in a home health setting
• Collaborate with interdisciplinary healthcare teams and paraprofessionals
• Support coordinated, patient-focused care within the community
• Maintain accurate documentation using Homecare Homebase (HCHB)

Qualifications

• Active Vermont RN License required
• Minimum 1 year of clinical nursing experience preferred
• Current BLS certification required
• Home Health experience preferred
• Experience with Homecare Homebase (HCHB) is a plus
