Job Description

We are seeking an experienced Outpatient Pharmacist for a travel assignment in a critical access healthcare facility. The ideal candidate will have outpatient infusion experience and strong clinical pharmacy skills in an outpatient setting. First-time travelers with the required experience are welcome to apply.

Key Responsibilities:

• Provide pharmaceutical care and medication management in an outpatient setting
• Support outpatient infusion therapy services
• Review medication orders for accuracy, safety, and compliance
• Counsel patients regarding medications, dosages, and treatment plans
• Collaborate with physicians, nurses, and interdisciplinary healthcare teams
• Maintain accurate documentation within the EMR system

Requirements:

• Active Pharmacist license required
• Minimum 1 year of outpatient pharmacist experience preferred
• Outpatient infusion experience required
• First-time travelers accepted
• Strong communication and clinical decision-making skills
• Experience with EPIC charting preferred
