About the Role:
Seeking a dedicated and experienced Licensed Practical Nurse (LPN) to support patient-centered care within a clinic setting.
Key Responsibilities:
Perform and document patient assessments within LPN scope of practice
Assist with patient health histories and admission processes
Support development and implementation of patient care plans
Monitor patient conditions and report changes appropriately
Administer medications, treatments, and nursing interventions safely
Maintain accurate and timely patient documentation
Assist with discharge planning and patient education
Communicate effectively with patients, families, and healthcare teams
Support patient education and follow-up care instructions
Maintain compliance with facility policies and standards of care
Requirements & Preferred Skills:
Active Minnesota LPN License required
Minimum 2 years of LPN experience preferred
Clinic or outpatient experience preferred
EPIC EMR experience preferred
Strong patient care and assessment skills
Excellent communication and interpersonal abilities
Ability to work effectively in team-based healthcare settings
Strong organizational skills and attention to detail
Ability to adapt in fast-paced clinical environments
