Location: Bronx, NY 10451
Duration: 14 Weeks
Schedule: 4 Days/Week | 38 Hours/Week
Shift: Day Shift – 8:00 AM to 4:30 PM

Position Overview

"Navitas Healthcare, LLC" is seeking an experienced Registered Nurse (RN) – Wound Care to provide specialized care in wound, ostomy, and continence management. This role involves conducting comprehensive patient assessments, developing individualized care plans, and collaborating with interdisciplinary teams to deliver high-quality patient outcomes.

Key Responsibilities

• Conduct detailed health assessments focused on wound, ostomy, and continence care
• Perform physical assessments and develop individualized treatment plans
• Monitor, document, and evaluate patient responses to treatments for wounds, fistulas, and related conditions
• Recommend appropriate medications, therapies, and interventions
• Collaborate with healthcare teams and initiate referrals when necessary
• Educate patients, families, and staff on prevention, care techniques, and treatment plans
• Develop and deliver staff education programs, including orientation and ongoing training
• Perform risk assessments and implement preventive measures for pressure injuries
• Participate in quality improvement initiatives, including audits and prevalence studies
• Ensure compliance with patient confidentiality and data security standards
• Assist in selecting wound care supplies, equipment, and technology
• Support asset management for specialty equipment (e.g., specialty beds, negative pressure devices)
• Address and escalate clinical or ethical concerns appropriately
• Promote culturally competent care aligned with patient values and beliefs

Required Qualifications

• Active New York State RN License (in good standing)
• Associate Degree in Nursing (ADN/ASN) from an accredited program
• Minimum 2 years of Med/Surg nursing experience
• Experience in wound care and colostomy management
• Current AHA BLS Certification

Required Certifications (One of the Following)

• CWOCN – Certified Wound Ostomy Continence Nurse
• CWON – Certified Wound Ostomy Nurse
• CWCN – Certified Wound Care Nurse
• CWS – Certified Wound Specialist
• WCC – Wound Care Certified
