Job Summary: We are seeking a detail-oriented Virtual Care Coordinator to support telehealth and remote patient monitoring initiatives within a dynamic healthcare environment. This role involves coordinating virtual care services, monitoring patient status remotely, and ensuring high-quality, patient-centered care through effective communication and collaboration with clinical and technical teams.

Key Responsibilities:

• Monitor patients remotely for safety and clinical needs
• Support hospital-based telemedicine services and virtual care workflows
• Assist in implementing telehealth and remote patient monitoring programs
• Collaborate with clinical teams, leadership, and technical partners
• Communicate effectively with patients, providers, and support teams
• Monitor hospital environmental status through virtual systems
• Assist in training coordination and telehealth best practice implementation
• Maintain accurate documentation in EMR systems
• Ensure adherence to quality, safety, and compliance standards

Required Qualifications:

• Minimum 2 years of telehealth experience (required)
• Experience in direct patient care
• Strong computer skills and familiarity with EMR systems
• Excellent organizational and multitasking abilities
• Strong attention to detail
• Effective verbal and written communication skills

Preferred Skills:

• Experience working with telehealth platforms and remote monitoring tools
• Ability to collaborate with cross-functional teams including clinical and IT
• Strong problem-solving and coordination skills
