Position Title: Registered Nurse – Case Manager
Location: Eureka, CA
Pay Rate (Local): $45.00 – $55.00 Per Hour
Pay Rate (Travel): $60.00 – $70.00 Per Hour
Weekly Hours: 40 Hours
Schedule: Night Shift – 5x8 Hours (07:00 PM – 07:00 AM) | Every Other Weekend
Duration: 13 Weeks (High Possibility of Extension)

Position Overview

"Navitas Healthcare, LLC" is seeking an experienced RN Case Manager to join a Case Management team within a leading acute care hospital in Eureka, CA. This role focuses on care coordination, discharge planning, utilization review, and ensuring appropriate use of healthcare services across the patient care continuum.

Primary Responsibilities

• Coordinate patient care and discharge planning across the continuum of care
• Conduct comprehensive patient needs assessments and develop individualized care plans
• Review admission criteria, insurance benefits, and prior authorization requirements
• Manage utilization review processes including appeals and denials
• Collaborate closely with interdisciplinary teams, including social services
• Ensure compliance with CMS, HIPAA, Joint Commission, and other regulatory standards
• Facilitate safe patient transitions and arrange durable medical equipment (DME) as needed
• Maintain accurate documentation of case management activities using Epic

Required Qualifications

• Minimum 2 years of acute hospital Case Management experience
• Recent experience within the last 12 months (required)
• Strong background in care coordination and discharge planning

Education

• Associate Degree in Nursing (ADN) or higher

Licensure / Certification

• Active Registered Nurse (RN) license – California
• BLS Certification (preferred)

Experience & Additional Requirements

• Experience with HMOs, IPAs, and managed care systems
• Knowledge of advance directives, POLST, and protective services processes
• Familiarity with CMS regulations, DRGs, and Joint Commission standards
• Willingness to float between units or facilities as needed
• Must provide 2 professional references covering at least 1 year of experience
• Experience with Epic charting system preferred
