Job Summary:

We are seeking an experienced Registered Nurse (RN) – Case Manager to join a dynamic healthcare team in Eureka, CA. This role focuses on coordinating patient care, ensuring appropriate resource utilization, and supporting positive patient outcomes in an acute care setting.

Key Responsibilities:

• Coordinate and manage patient care plans across the continuum of care
• Collaborate with physicians, nurses, and interdisciplinary teams to ensure effective treatment and discharge planning
• Perform utilization review and ensure appropriate level of care
• Facilitate timely and safe patient discharges
• Communicate with patients and families regarding care plans and available resources
• Maintain accurate and timely documentation in EPIC EMR system
• Monitor patient progress and adjust care plans as needed

Work Schedule & Requirements:

• Night shift schedule (5x8-hour shifts)
• Weekend Requirement: Every other weekend (EOW)
• No on-call requirement
• Floating: May be required between like units based on facility needs

Required Qualifications:

• Active California RN License (required at time of submission)
• Minimum 2 years of acute care experience (required)
• Case Management experience preferred
• Experience with EPIC EMR system
• Strong care coordination, communication, and organizational skills
