Licensed Practical Nurse (LPN/LVN) – Long-Term Care (LTC)

Location: Grand Junction, CO 81506
Duration: 13-Week Contract
Schedule: 3x12-Hour Variable Night Shifts
Guaranteed Hours: 48 hours/week

Job Overview

A healthcare facility is seeking experienced LPN/LVNs for a Long-Term Care (LTC) / Nursing Home setting. The ideal candidate will be comfortable working night shifts and delivering high-quality patient care in a fast-paced, team-oriented environment.

Key Responsibilities

• Provide direct patient care in a long-term care setting
• Administer medications and treatments as prescribed
• Monitor resident conditions and promptly report any changes
• Perform wound care and basic nursing procedures
• Assist residents with activities of daily living (ADLs) and ensure comfort
• Maintain accurate documentation and patient records
• Collaborate with RNs, CNAs, and interdisciplinary care teams

Required Qualifications

• Active LPN/LVN license (Colorado or compact state)
• Minimum 1 year of experience in LTC or nursing home setting
• Strong clinical and patient care skills
• Ability to work independently during night shifts

Assignment Details

• Shift Type: Variable Night Shifts (12 hours)
• Weekly Hours: 48 (Guaranteed)
