Job Overview

"Navitas Healthcare, LLC" is seeking a dedicated and experienced Licensed Practical Nurse (LPN) for a clinic setting. The LPN will provide patient-centered care, support clinical assessments, and collaborate with the healthcare team to deliver high-quality outpatient services.

Key Responsibilities

• Perform and document patient assessments within LPN scope of practice
• Assist with patient health histories and admission processes
• Identify patient care needs and support care planning with the clinical team
• Implement and monitor care plans under the direction of licensed practitioners
• Observe, report, and respond to changes in patient condition
• Administer medications, treatments, and nursing interventions safely and accurately
• Maintain timely and accurate clinical documentation
• Support discharge planning and patient education initiatives
• Communicate effectively with patients, families, and interdisciplinary teams
• Ensure proper task delegation while maintaining patient safety standards
• Follow all facility policies, procedures, and clinical guidelines

Required Qualifications

• Active Minnesota LPN License (required at submission)
• Minimum 2 years of LPN experience, preferably in clinic or outpatient settings
• Experience with EPIC EMR preferred
• Strong knowledge of clinical procedures and patient care practices
• Excellent communication and interpersonal skills
• Ability to work in a fast-paced, team-oriented environment
• Strong organizational skills and attention to detail
