Shift: Evening or Night Shift – 5x8 Hours

• Evening: 15:00 – 23:30
• Night: 23:00 – 07:30

Assignment Duration: 8 Weeks

Job Description:

We are seeking a dedicated LVN/LPN for a long-term care and rehabilitation setting. This role involves providing quality nursing care to a diverse patient population, including both long-term residents and short-term rehab patients.

Key Responsibilities:

• Deliver direct patient care in a skilled nursing facility
• Monitor patient conditions and perform blood glucose testing as needed
• Administer medications and treatments per physician orders
• Maintain accurate documentation using electronic medical records
• Collaborate with interdisciplinary teams to ensure quality patient outcomes
• Float between units within the facility as required

Requirements:

• Active LPN/LVN license
• Minimum 1 year of LTC (long-term care) experience required
• BLS certification required
• Experience in skilled nursing facility or nursing home setting
• Proficiency in blood glucose monitoring/testing
• Experience with Epic charting system required
