Licensed Master Social Worker (LMSW) – Case Management (Acute Care)

Location: Everett, Washington
Shift: Day Shift – 5x8 (07:30 AM – 04:00 PM)
Schedule: Every other weekend required | Floating between hospital units and campuses (within ~2 miles)

Position Overview

"Navitas Healthcare, LLC" is seeking an experienced Licensed Master Social Worker (LMSW) to support an acute care case management team. The role focuses on delivering comprehensive social work services, discharge planning, and care coordination across multiple hospital units and campuses.

Key Responsibilities

• Provide case management and care coordination for hospitalized patients
• Conduct psychosocial assessments and develop effective discharge plans
• Facilitate safe and timely transitions of care
• Collaborate with interdisciplinary teams including physicians, nursing staff, and therapy departments
• Support patients and families with counseling, resources, and community referrals
• Maintain accurate and timely documentation in EPIC EHR
• Float between multiple units and campuses as operationally required
• Work every other weekend as part of the rotating schedule

Requirements

• Active Washington State LMSW license (required)
• Minimum 1 year of acute care social work experience
• Strong experience in discharge planning and care coordination
• Background in Med/Surg, ED, or long-length-of-stay populations preferred
• BLS certification required
• Ability to work in a fast-paced, multi-unit hospital environment
