Shift: Evening Shift
Schedule: 5 × 8-hour shifts per week
Hours: 37.5–40 hours/week
Weekend Requirement: Every other weekend
Holiday Requirement: Every other holiday

Job Description:

We are seeking a dedicated Licensed Practical Nurse (LPN) to provide high-quality patient care in a long-term care (LTC) and skilled nursing setting. The LPN will support residents by delivering compassionate care, assisting with daily clinical tasks, and collaborating with the healthcare team to ensure optimal outcomes.

Responsibilities:

• Provide direct patient care under the supervision of an RN or physician
• Administer medications and treatments as prescribed
• Monitor and document patient conditions and report changes promptly
• Assist with daily living activities and support resident comfort
• Maintain accurate and timely documentation using electronic systems
• Collaborate with nursing staff and interdisciplinary teams
• Support and assist with CNA duties as needed
• Ensure compliance with facility policies, safety, and infection control standards

Requirements:

• Active LPN license (State-specific)
• Recent experience in a long-term care or nursing home setting required
• Experience with PointClickCare (PCC) required
• Strong communication and teamwork skills
• Ability to work in a fast-paced environment and manage multiple priorities
