Schedule: Full-Time (Day/Night Shifts Available)


Position Summary

The RN – Long Term Care is responsible for delivering comprehensive, patient-centered nursing care to residents across all age groups. This role includes clinical decision-making, care coordination, patient education, leadership responsibilities, and supervision of support staff within a long-term care setting.


Key Responsibilities

Clinical Care

  • Provide safe, therapeutic, and age-appropriate nursing care
  • Assess, plan, implement, and evaluate resident care plans
  • Administer medications and treatments accurately and safely
  • Respond promptly to emergencies and changes in patient condition
  • Document assessments, interventions, and outcomes in the electronic health record (EHR)

Care Coordination

  • Collaborate with physicians and interdisciplinary healthcare teams
  • Coordinate resident care activities and treatment plans
  • Communicate effectively with residents and their families

Leadership & Supervision

  • Supervise CNAs, LPNs, technicians, and support staff during assigned shifts
  • Serve as charge nurse when required
  • Delegate tasks and provide clinical guidance to team members
  • Participate in staff training, education, and mentoring

Patient & Family Support

  • Educate residents and families on conditions, treatments, and care plans
  • Support psychosocial needs and coping strategies
  • Participate in care conferences and discharge planning

Quality & Compliance

  • Adhere to infection control and patient safety protocols
  • Maintain accurate and confidential documentation
  • Participate in staff meetings and ongoing training
  • Ensure compliance with organizational policies and state regulations

Requirements

Education

  • Graduate of an accredited nursing program (ADN, BSN, or Diploma)
  • High school diploma or equivalent

Experience

  • 1–2 years of nursing experience preferred

Licensure & Certifications

  • Active RN license (or ability to obtain prior to start)
  • Basic Life Support (BLS) certification
  • Clinical competency within 3 months of hire (renewed every 3 years)
  • High-risk competency within 3 months of hire (renewed annually)
