Position Title: Licensed Practical Nurse (LPN) – Long Term Care (Night Shift)

Location: Gunnison, Colorado
Duration: 13 Weeks
Setting: Long-Term Care


Schedule

  • Night Shift
  • 12-hour shifts
  • 36 hours guaranteed per week
  • Overtime applicable after 40 hours/week
  • Holiday coverage required

Position Overview

"Navitas Healthcare, LLC" is experienced and compassionate Licensed Practical Nurses (LPNs) to support a Long-Term Care unit. The LPN will provide high-quality nursing care, oversee daily patient care activities, and ensure compliance with clinical standards and regulatory requirements to maintain optimal resident outcomes.


Key Responsibilities

  • Provide direct nursing care to residents in accordance with individualized care plans
  • Supervise and support daily nursing activities during assigned shifts
  • Administer medications, treatments, and IV therapy (if certified) as per provider orders
  • Monitor resident conditions and promptly report changes to RN or physician
  • Assist with rehabilitative and restorative nursing practices
  • Maintain accurate and timely documentation in PointClickCare EMR
  • Ensure compliance with facility policies, state regulations, and federal guidelines
  • Participate in care planning, shift reporting, and interdisciplinary communication
  • Maintain infection control standards and ensure resident safety
  • Respond effectively to emergencies and provide appropriate interventions

Required Qualifications

  • Active Licensed Practical Nurse (LPN) License – Colorado
  • Minimum 3 years of nursing experience in hospital, SNF, or similar setting
  • BLS (Basic Life Support) certification

Preferred Qualifications

  • IV Certification
  • Experience in Long-Term Care or Skilled Nursing Facility
  • Knowledge of rehabilitative and restorative nursing practices
