Job Description:

We are seeking an experienced Medical Social Worker (MSW) to support patient care in an acute care setting. The ideal candidate will play a key role in care coordination, discharge planning, and providing psychosocial support to patients and their families.

Key Responsibilities:

• Provide psychosocial assessments and support to patients and families
• Assist with discharge planning and coordination of post-acute care services
• Collaborate with interdisciplinary healthcare teams
• Manage a caseload of approximately 20–23 patients
• Connect patients with community resources and support services
• Maintain accurate and timely documentation

Required Experience & Skills:

• Minimum 1 year of acute care social work experience
• Prior travel experience required
• Strong knowledge of care coordination and discharge planning
• Ability to manage multiple cases in a fast-paced environment
• Excellent communication and interpersonal skills

Qualifications:

• Master’s Degree in Social Work (MSW) – Required
• Active state license (preferred; must be active by start date if required)
• Strong organizational and problem-solving skills
