Job Title: LPN / LVN
Location: Watertown, NY
Duration: 13 Weeks
Schedule: Full-Time | 40 hours/week (5x8)
Shift: Day Shift (8-hour shifts)

Position Overview

A well-established healthcare facility is seeking a compassionate and dependable Licensed Practical Nurse (LPN/LVN) to provide quality care in a Long Term Care (LTC) setting. The ideal candidate will support residents with chronic conditions while ensuring comfort, safety, and continuity of care.

Responsibilities

  • Provide direct nursing care to residents in a long-term care environment
  • Administer medications and treatments as prescribed
  • Monitor and document patient conditions and report changes to the healthcare team
  • Assist with daily living activities and support patient comfort
  • Utilize electronic medical records (eMAR – PointClickCare) for documentation
  • Collaborate with RNs, physicians, and care teams to deliver patient-centered care
  • Ensure compliance with facility policies and patient safety standards

Patient Population

  • Dementia
  • Chronic diseases
  • Diabetes

Experience Requirements

  • 1–3 years of LPN/LVN experience required
  • Experience in Long Term Care (LTC) or Skilled Nursing Facility (SNF) preferred

Licensure & Certifications

  • Active New York State LPN/LVN license required
  • Basic Life Support (BLS) certification from the American Heart Association (AHA) required
