Position Summary

"Navitas Healthcare, LLC" is a dedicated Licensed Practical Nurse (LPN) to provide high-quality care in an evening shift Long-Term Care (LTC) setting. The LPN will deliver hands-on nursing care, collaborate with interdisciplinary teams, and ensure resident safety, comfort, and well-being in a skilled nursing environment.

Key Responsibilities

• Administer medications and treatments as ordered by physicians
• Provide direct patient care including assistance with bathing, feeding, toileting, and mobility
• Monitor, document, and report resident vital signs, conditions, and responses to care
• Collaborate with RNs, CNAs, and healthcare team members to ensure resident-centered care
• Assist in developing and implementing individualized care plans
• Maintain infection control and safety standards at all times
• Support resident safety and serve as a companion when needed
• Participate in quality improvement initiatives to enhance resident outcomes
• Maintain accurate and timely clinical documentation

Qualifications

• Active LPN license in Wisconsin
• Experience in Long-Term Care or Skilled Nursing preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication and teamwork abilities
