Job Title: Licensed Practical Nurse (LPN) – Long-Term Care (Days)

Location: Milbank, SD
Facility Type: Long-Term Care / Nursing Home
Shift: Day Shift (Variable 12-hour shifts)
Schedule: 36 hours/week

Position Summary

"Navitas Healthcare, LLC" is a skilled and compassionate Licensed Practical Nurse (LPN) to join a Long-Term Care (LTC) team. This role provides the opportunity to deliver quality nursing care in a supportive and resident-focused environment.

Key Responsibilities

• Provide direct nursing care to residents, including medication administration and treatments as prescribed
• Monitor, document, and report resident conditions, vital signs, and progress
• Assist in developing and implementing individualized care plans
• Collaborate with RNs, CNAs, and other healthcare team members
• Maintain compliance with facility policies, procedures, and state regulations
• Educate residents and families regarding care plans, wellness, and health maintenance
• Participate in quality improvement initiatives to improve resident outcomes
• Maintain accurate and timely documentation

Qualifications

• Active LPN license in South Dakota
• Experience in Long-Term Care preferred
• Strong clinical judgment and critical thinking skills
• Good communication and teamwork abilities
