Job Summary:

We are seeking an experienced Social Worker to provide support services in an acute hospital setting. The ideal candidate will work with patients and families to address psychosocial needs, coordinate care, and assist with discharge planning.

Key Responsibilities:

• Provide psychosocial assessments and support to patients and families
• Develop and implement care and discharge plans
• Coordinate with healthcare teams to ensure continuity of care
• Assist with crisis intervention and resource planning
• Maintain accurate and timely documentation
• Ensure compliance with hospital policies and regulatory standards

Qualifications:

• Active Licensed Clinical Social Worker (LCSW) license (state-specific)
• Minimum of 2 years of experience in a hospital or acute care setting
• Strong communication and interpersonal skills
• Ability to work in a fast-paced healthcare environment
