Job Overview

The Licensed Practical Nurse (LPN) in a long-term care setting provides direct patient care to residents, ensuring their safety, comfort, and overall well-being. The LPN works under the supervision of a Registered Nurse and collaborates with the healthcare team to deliver high-quality, patient-centered care.


Key Responsibilities

  • Provide direct nursing care to residents in a long-term care facility
  • Administer medications and treatments as prescribed
  • Monitor and record vital signs and patient conditions
  • Assist residents with activities of daily living (ADLs)
  • Perform wound care and other clinical procedures
  • Maintain accurate and timely patient documentation
  • Report changes in patient condition to the RN or physician
  • Ensure compliance with infection control and safety protocols
  • Support care plan implementation and updates
  • Respond promptly to patient needs and emergencies

Qualifications

Education

  • Diploma or certification from an accredited Practical Nursing program
    or High School Diploma

Licensure and Certification

  • Active LPN/LVN License (required)
  • Basic Life Support (BLS) certification (required)

Experience and Requirements

  • Minimum 1 year of LPN experience in long-term care or skilled nursing facility preferred
  • Familiarity with EMR systems
  • Ability to work flexible shifts, including nights, weekends, and holidays
  • Knowledge of geriatric care and chronic conditions
  • Strong time management and organizational skills
  • Ability to work in a fast-paced environment
  • Team collaboration and adaptability
