Job Title: Registered Nurse (RN) – Long-Term Care
Location: Berlin, New Hampshire
Shift: Day Shift
Schedule: 12-hour shifts
Guaranteed Hours: 36 hours per week

Job Summary

"Navitas Healthcare, LLC" is seeking a compassionate and experienced Registered Nurse (RN) to provide high-quality patient care in a long-term care and rehabilitation setting. The RN will be responsible for monitoring patient health, administering treatments, coordinating care plans, and ensuring a safe and supportive environment for residents.

Key Responsibilities

• Administer medications and treatments in accordance with physician orders and facility protocols
• Monitor residents’ conditions, assess changes, and report updates to the healthcare team
• Develop, implement, and update individualized patient care plans
• Supervise and collaborate with LPNs, CNAs, and other support staff
• Maintain accurate and timely patient documentation
• Ensure compliance with clinical standards, safety policies, and regulatory requirements
• Provide compassionate support and education to residents and their families

Qualifications

• Active Registered Nurse (RN) license in New Hampshire
• Previous Long-Term Care (LTC) or rehabilitation experience preferred
• Strong clinical assessment and documentation skills
• Ability to work effectively in a collaborative healthcare environment
