Position Title: Travel Licensed Practical Nurse (LPN)
Location: Saint Albans, Vermont
Job Type: Contract (Travel Assignment)
Weekly Hours: 36–40 hours
Schedule: 8-hour and 12-hour shifts available (Day, Evening & Night shifts)


Position Overview

"Navitas Healthcare, LLC" is seeking a dedicated and compassionate Travel Licensed Practical Nurse (LPN) to join a Skilled Nursing Facility (SNF) in Saint Albans, Vermont. The LPN will play a key role in delivering high-quality patient care, ensuring resident comfort, safety, and adherence to individualized care plans. This position is ideal for professionals who thrive in a fast-paced, team-oriented healthcare environment.


Key Responsibilities

  • Provide direct patient care in accordance with established care plans
  • Administer medications and treatments as prescribed
  • Monitor patient conditions and promptly report changes to the RN or physician
  • Assist residents with activities of daily living, including hygiene, mobility, and feeding
  • Maintain accurate and timely documentation of patient records
  • Follow infection control protocols and safety guidelines
  • Collaborate with interdisciplinary teams to optimize patient outcomes

Required Qualifications

  • Active LPN license in the state of Vermont or compact state
  • Minimum 1–2 years of experience in a Skilled Nursing Facility (SNF) or similar setting
  • Strong clinical and patient care skills
  • Ability to adapt quickly in a travel assignment environment

Education

  • Diploma or Certificate in Practical Nursing from an accredited institution

Certifications & Documentation

  • Current LPN License
  • AHA BLS Certification
  • COVID-19 Vaccination (as required)
  • Updated Resume
  • Valid Driver’s License

Preferred Experience

  • Experience in long-term care, rehabilitation, or SNF settings
  • Familiarity with Electronic Medical Records (EMR) systems
