Compensation & Benefits:

• Salary Range (based on experience):
  • 0–3 years: $102,107 – $115,000
  • 3+ years: $115,000 – $127,587
  • 5+ years: $127,587 – $140,000
• Sign-On Bonus: $10,000
• Relocation Assistance: Available
• Schedule: Primarily weekdays (occasional on-call or extended hours may be required)

Job Summary:

We are seeking an experienced Director of Case Management to oversee and coordinate all departmental operations within a dynamic acute care hospital environment. This leadership role ensures effective care coordination, discharge planning, regulatory compliance, and integration of patient services to optimize outcomes and continuity of care.

Responsibilities:

• Lead and manage all case management operations, including staffing, workflows, and care delivery models
• Ensure compliance with organizational policies, regulatory requirements, and industry best practices
• Oversee care coordination and discharge planning to ensure quality, efficiency, and timeliness
• Manage departmental resources, including staffing levels (FTEs) and productivity targets
• Analyze performance metrics and implement process improvements to enhance patient outcomes
• Collaborate with executive leadership and interdisciplinary teams to align strategic goals
• Develop and maintain strong relationships with internal departments and external stakeholders
• Drive quality improvement initiatives and operational excellence
• Ensure accurate documentation and adherence to standards of practice

Qualifications:

Education:

• Bachelor’s Degree (required)
• Master’s Degree in Nursing, Healthcare Administration, or Business Administration (preferred)

Licensure:

• Active RN, LCSW, or LMSW license (required)

Experience:

• Minimum 3+ years of acute care hospital case management experience (required)
• Minimum 2+ years of leadership experience in case management (required)
