Compensation: $117,423 – $170,000 annually (DOE) + 17.5% Bonus
Sign-On Bonus: $10,000 (for external hires)

Compensation Breakdown

  • 0–3 years experience: $117,423 – $130,000

  • 3+ years experience: $130,000 – $146,725

  • 5+ years experience: $146,725 – $155,000

  • 10–15+ years experience: Up to $170,000

Position Summary

The Director of Case Management provides leadership and operational oversight for the case management department, ensuring compliance with regulatory standards and driving efficient care coordination and discharge planning. This role partners with interdisciplinary teams and executive leadership to optimize patient care, resource utilization, and departmental performance.

Key Responsibilities

  • Lead daily case management operations and staff

  • Develop and implement departmental strategy, staffing, and workflows

  • Ensure compliance with care coordination and discharge planning standards

  • Monitor and improve departmental performance using data-driven insights

  • Collaborate with executive leadership (CFO, CMO, CNO)

  • Manage staffing models, productivity, and resource allocation

  • Drive quality improvement and performance initiatives

  • Build and maintain relationships with internal and external stakeholders

  • Ensure adherence to organizational policies, SOPs, and regulatory requirements

  • Mentor interdisciplinary teams to achieve operational excellence

Required Qualifications

  • Bachelor’s Degree (Required)

  • Master’s Degree in Nursing, Healthcare Administration, or Business Administration (Preferred)

  • Active RN, LCSW, or LMSW license (Required)

  • 3+ years of acute care hospital case management experience (Required)

  • 2+ years of case management leadership experience (Preferred)

  • Recent acute care experience (within the last year)
