Position Title: Licensed Practical Nurse (LPN)
Location: Falmouth, MA 02540
Pay Rate: $45.00 – $60.00/hour (Based on Experience)
Schedule: Full-Time | 40 Hours/Week
Day & Evening Shifts Available
7:00 AM – 3:30 PM
3:00 PM – 11:30 PM
Every other weekend and holiday required
The Licensed Practical Nurse (LPN) provides direct patient care in a skilled nursing setting while supporting and supervising nursing assistants. This role ensures high-quality care delivery in compliance with all applicable regulations and standards.
Supervise and support nursing assistants in daily care activities
Administer medications and treatments as prescribed
Monitor patient conditions and document care accurately
Implement and transcribe physician orders
Assist with admissions, transfers, and discharges
Perform clinical procedures (wound care, catheterization, tube feeding, etc.)
Collect and process lab specimens
Record vital signs and patient observations
Communicate effectively with physicians and care teams
Participate in care planning and interdisciplinary coordination
Educate patients and families on care plans
Ensure adequate medical supplies and equipment
Maintain patient confidentiality and regulatory compliance
Respond promptly to patient needs ensuring safety and comfort
Education:
Graduate of an accredited Practical Nursing program
Licensure & Certifications:
Active LPN License (Massachusetts or Compact) – Required
BLS / CPR / AED Certification – Required
Experience in long-term care or skilled nursing preferred
Strong clinical and patient care skills
Effective communication and teamwork abilities
Leadership and supervisory experience preferred
Strong documentation and organizational skills
Ability to work in a fast-paced environment and handle emergencies
