Position Title: Licensed Practical Nurse (LPN)
Location: Falmouth, MA 02540
Pay Rate: $45.00 – $60.00/hour (Based on Experience)
Schedule: Full-Time | 40 Hours/Week

Shift Details

  • Day & Evening Shifts Available

  • 7:00 AM – 3:30 PM

  • 3:00 PM – 11:30 PM

  • Every other weekend and holiday required


Position Overview

The Licensed Practical Nurse (LPN) provides direct patient care in a skilled nursing setting while supporting and supervising nursing assistants. This role ensures high-quality care delivery in compliance with all applicable regulations and standards.


Key Responsibilities

  • Supervise and support nursing assistants in daily care activities

  • Administer medications and treatments as prescribed

  • Monitor patient conditions and document care accurately

  • Implement and transcribe physician orders

  • Assist with admissions, transfers, and discharges

  • Perform clinical procedures (wound care, catheterization, tube feeding, etc.)

  • Collect and process lab specimens

  • Record vital signs and patient observations

  • Communicate effectively with physicians and care teams

  • Participate in care planning and interdisciplinary coordination

  • Educate patients and families on care plans

  • Ensure adequate medical supplies and equipment

  • Maintain patient confidentiality and regulatory compliance

  • Respond promptly to patient needs ensuring safety and comfort


Required Qualifications

Education:

  • Graduate of an accredited Practical Nursing program

Licensure & Certifications:

  • Active LPN License (Massachusetts or Compact) – Required

  • BLS / CPR / AED Certification – Required


Experience & Skills

  • Experience in long-term care or skilled nursing preferred

  • Strong clinical and patient care skills

  • Effective communication and teamwork abilities

  • Leadership and supervisory experience preferred

  • Strong documentation and organizational skills

  • Ability to work in a fast-paced environment and handle emergencies
