Shift: Multiple Openings – Day, Evening, and Night
Pay Rate: $80 – $90 per hour (based on experience)
Estimated Weekly Gross Pay: $3,200 – $3,600
Weekly Stipend: $1,561.20
A healthcare facility in Hyannis, MA is seeking an experienced RN Case Manager / Clinical Resource Manager to oversee the daily operations of Case Management, Utilization Review, Social Work, and Clinical Documentation teams.
This role focuses on discharge planning, utilization management, patient care coordination, and departmental leadership while ensuring compliance with healthcare regulations and supporting continuous process improvement initiatives.
Oversee daily operations of Case Management and Social Services teams, including staff scheduling and performance management
Conduct regular patient unit rounds to identify barriers to discharge and support care coordination
Develop and deliver continuing education programs for Case Managers and Social Workers
Provide training and orientation for new clinical staff, including nurses, physicians, social workers, and allied health professionals
Monitor program compliance and performance metrics and develop dashboards to track departmental outcomes
Lead process improvement initiatives to enhance efficiency and patient care coordination
Facilitate collaboration between physicians, nurses, case managers, and the clinical resource team
Analyze operational data and ensure compliance with regulatory and accreditation standards
Identify workflow challenges and implement solutions to improve departmental performance
Bachelor’s Degree in Nursing (BSN) required (Master’s Degree preferred)
Active Massachusetts Registered Nurse (RN) License
Minimum 5 years of Case Management experience in an acute care setting
At least 2 years of process improvement experience
Knowledge of CMS regulations and healthcare compliance standards
Strong analytical, leadership, and communication skills
Case Management Certification (CCM) or certification in Clinical Documentation Integrity (CDI)
Previous management or supervisory experience
Experience developing and presenting continuing education programs
