Pay Rate: $50 – $56 per hour (Travel Rate | Based on Experience)


Shift & Schedule

  • Primary Shift: Evening Shift (3:00 PM – 11:00 PM)

  • Occasional Shift: Day Shift (7:00 AM – 3:00 PM)

  • Hours: 40 Hours per Week

  • Schedule: 5 × 8-Hour Shifts

  • Rotation: Every Other Weekend & Holiday


Position Overview

"Navitas Healthcare, LLC" is seeking a dedicated Licensed Practical Nurse (LPN) to provide quality nursing care in a long-term care setting. The LPN will support resident health and wellbeing by administering medications, performing treatments, documenting care, and coordinating with physicians and care teams while supervising nursing assistants during assigned shifts.


Key Responsibilities

Resident Care

  • Provide direct nursing care according to physician orders and care plans

  • Administer medications and monitor residents for reactions or changes

  • Monitor and document vital signs (temperature, pulse, respiration, blood pressure)

  • Perform treatments including wound care, catheter care, tube feeding, suctioning, and dressing changes

  • Assist residents with therapeutic and mobility-related care

Nursing Supervision

  • Supervise and guide nursing assistants during assigned shifts

  • Coordinate daily care activities with nursing and support staff

  • Ensure adherence to facility policies and nursing standards

Documentation

  • Maintain accurate patient charts and nursing documentation

  • Record resident conditions, treatments, and responses to care

  • Update medication administration records and treatment plans

  • Document admissions, transfers, and discharges as required

Care Coordination

  • Communicate with physicians regarding patient status and treatment plans

  • Receive and document physician orders

  • Participate in care discussions and provide updates during rounds

Clinical & Laboratory Support

  • Collect laboratory specimens such as blood, urine, and sputum

  • Assist with routine laboratory testing and documentation of results

Resident & Family Support

  • Educate residents and families regarding care plans and health needs

  • Support resident orientation and adjustment to the facility environment

Compliance & Operations

  • Maintain confidentiality of resident health information (HIPAA compliance)

  • Support regulatory surveys and facility inspections when required

  • Ensure adequate availability of medications and nursing supplies


Required Qualifications

Education

  • Graduate of an accredited Licensed Practical Nurse (LPN) program

License

  • Active and unrestricted Massachusetts LPN License

Certifications

  • BLS / CPR / AED Certification

Experience

  • Experience in Long-Term Care or Skilled Nursing preferred

  • Experience with medication administration and patient charting


Skills & Competencies

  • Strong clinical and patient care skills

  • Ability to supervise and support nursing assistants

  • Excellent documentation and electronic charting skills

  • Strong communication with residents, families, and healthcare teams

  • Ability to work independently and make sound clinical decisions

  • Ability to lift up to 50 lbs and meet physical job requirements
