Job Title: Home Health Registered Nurse (RN)
Location: South Dennis, MA 02660
Schedule: Day/Evening Shift | Monday – Friday
Compensation: $2,400 – $2,800 Weekly Gross (Based on Experience)
We are seeking an experienced and compassionate Home Health Registered Nurse (RN) to provide skilled nursing services in a community-based setting in South Dennis, MA. The RN will be responsible for delivering high-quality, patient-centered care while ensuring compliance with clinical standards, regulatory guidelines, and documentation requirements.
This role requires strong assessment skills, independent clinical judgment, and effective coordination with an interdisciplinary healthcare team.
Maintain annual HHRG values in alignment with national standards.
Utilize HHRG values to determine appropriate visit frequency and interdisciplinary resource utilization.
Ensure patient outcomes meet established national benchmarks.
Assess patient clinical needs and develop, implement, and revise individualized plans of care.
Address all identified problems within the plan of care and document accordingly.
Participate in interdisciplinary case conferences and evaluate care plan effectiveness.
Provide instruction and documented supervision of the Home Health Aide plan of care every 14 days.
Notify physicians and other healthcare providers of significant changes in patient condition.
Complete admissions and required documentation in a timely and accurate manner.
Maintain clear, concise narrative documentation reflecting skilled nursing care and patient/caregiver response.
Obtain and clarify valid physician orders for all delivered services.
Plan and coordinate appropriate discharges, including documentation of disposition in discharge summaries.
Refer patients to community resources when appropriate.
Current Registered Nurse (RN) license in Massachusetts.
Successful completion of VNA pre-hire medication test.
Minimum one (1) year of nursing experience required, or six (6) months with a BSN.
Current CPR certification.
