Job Title: Home Health Registered Nurse (RN)
Location: Haven, MA 02568
Assignment Duration: 13 Weeks
Schedule: Monday – Thursday (Primary) | Option for 40 hours Monday – Friday if needed
Weekend Rotation: Approximately every 4 weeks
Pay Rate: $65 – $75 per hour (based on experience)
We are seeking a compassionate and experienced Home Health Registered Nurse (RN) for a 13-week contract assignment in Haven, MA. The RN will provide skilled nursing services in a home care setting, ensuring high-quality, patient-centered care while maintaining compliance with regulatory and organizational standards.
This role requires strong clinical assessment skills, care coordination expertise, and the ability to work independently in a community-based environment.
Evaluate patient clinical needs and develop, implement, and revise individualized plans of care.
Maintain appropriate Home Health Resource Group (HHRG) values aligned with national standards.
Establish visit frequency and coordinate interdisciplinary services as needed.
Participate in interdisciplinary case conferences and evaluate care effectiveness.
Provide instruction and documented supervision of the Home Health Aide plan of care every 14 days.
Notify physicians and care providers of significant changes in patient condition.
Obtain and clarify valid physician orders for all care delivered.
Complete admissions, documentation, and required paperwork in a timely manner.
Maintain clear, concise narrative documentation reflecting skilled nursing care and patient/caregiver response.
Coordinate and document appropriate discharge planning and summaries.
Refer patients to community resources when appropriate.
Current Registered Nurse (RN) license in Massachusetts.
Successful completion of VNA pre-hire medication test.
Minimum 1 year of nursing experience required, or 6 months with a BSN.
Current CPR certification.
