Pay Rate: $60 – $63 per hour
Shift: Day Shift (5x8 schedule)
Hours: 40 hours per week
Weekend Requirement: Every other weekend
On-Call/Holidays: No on-call or holiday requirement
"Navitas Healthcare, LLC" is seeking a compassionate and skilled Physical Therapist (PT) to provide high-quality rehabilitation services to patients in their homes. The PT will be responsible for evaluating patients, developing individualized treatment plans, implementing therapy programs, and ensuring goal-oriented care in compliance with regulatory and organizational standards.
This position offers a consistent weekday schedule with a rotating weekend requirement and provides the opportunity to deliver meaningful, patient-centered care in a home health setting.
Perform comprehensive and timely patient evaluations.
Develop measurable short- and long-term treatment goals.
Implement individualized therapy interventions using appropriate clinical techniques.
Complete accurate and timely documentation, including progress notes, reassessments, and discharge summaries.
Ensure documentation supports medical necessity for continued skilled home care services.
Follow physician-ordered plans of care, including treatment frequency and duration.
Supervise and collaborate with Physical Therapist Assistants (PTAs) in compliance with regulatory standards.
Educate patients and caregivers on home exercise programs and care plans.
Participate in discharge planning and interdisciplinary care coordination.
Maintain effective communication with physicians and care team members.
Meet established productivity and quality benchmarks.
Perform physical tasks including lifting, pushing, and pulling up to 50 lbs., and standing for extended periods.
Bachelor’s Degree in Physical Therapy.
Active State Physical Therapist License (NJ).
AHA Basic Life Support (BLS) Certification.
Minimum 1 year of Physical Therapy experience.
Certified Home Care (Part A) experience with current OASIS knowledge and usage.
Strong clinical assessment and intervention skills.
Proficiency in electronic documentation systems.
Excellent communication and multitasking abilities.
Doctor of Physical Therapy (DPT).
Prior home health experience strongly preferred.
Experience with EPIC electronic medical records system.
