A healthcare facility is seeking a dedicated Licensed Practical Nurse (LPN) for a Long-Term Care (LTC) contract assignment. The LPN will provide high-quality, compassionate nursing care to residents in a long-term care setting, supporting patient safety, comfort, and overall well-being.
Administer medications and treatments as prescribed
Monitor resident health status and report changes to the care team
Perform wound care and basic nursing procedures
Measure and document vital signs and patient observations
Assist residents with activities of daily living (ADLs)
Supervise and support Certified Nursing Assistants (CNAs)
Maintain accurate and timely documentation
Implement and follow individualized resident care plans
Ensure a safe, clean, and supportive care environment
Comply with facility policies, procedures, and nursing standards
Active LPN license (New York or Compact, if applicable)
Long-Term Care or Skilled Nursing experience preferred
Ability to work night shifts and adapt to schedule needs
Strong communication, teamwork, and patient-care skills
Commitment to resident safety and quality care
