Schedule: Monday–Friday | 8:30 AM – 5:00 PM
Pay Rate: $40–$42/hour

Role Summary

"Navitas Healthcare, LLC" is seeking a Hospital-Based Social Worker (LMSW) to provide comprehensive psychosocial assessments, counseling, and discharge planning services for patients and families. This role supports individuals facing medical, emotional, financial, and environmental challenges that may impact care outcomes. Assignments may include medical, renal, and transplant units and are not limited to adult psychiatric services.

Responsibilities

  • Conduct psychosocial assessments to evaluate patient and family needs

  • Provide individual and group counseling related to medical or psychiatric care

  • Coordinate discharge planning, including referrals to rehabilitation and extended care facilities

  • Connect patients and families with community resources, financial assistance programs, and support services

  • Collaborate with multidisciplinary healthcare teams and participate in care conferences

  • Document assessments, care plans, and interventions in the electronic medical record (Epic preferred)

  • Support quality improvement initiatives, education, and departmental activities

  • Maintain current knowledge of community-based and government social service programs

  • May provide supervision to junior staff or students, depending on program requirements

Required Qualifications

  • Licensed Master Social Worker (LMSW) – required (LCSW may be required for certain units)

  • Master’s degree in Social Work from an accredited institution

  • 6 months to 1 year of experience in a medical or healthcare setting preferred

  • Strong interpersonal, counseling, and analytical skills

  • Ability to work effectively with patients and families in high-stress environments

  • Experience with Epic EMR preferred

  • Basic proficiency in Microsoft Office applications
