Location: Bronx, NY 10461
Contract Duration: 13 Weeks
Schedule: 40 Hours/Week | Day Shift

Weekly Gross: $2,779.87

Job Summary
This role focuses on care coordination and discharge planning in a fast-paced Emergency Department to promote timely transitions and efficient patient flow.

Responsibilities

• Coordinate patient care across ED services
• Conduct utilization reviews and discharge planning
• Collaborate with interdisciplinary teams
• Identify discharge needs and arrange services
• Ensure regulatory and payer compliance

Qualifications

• NY State RN license
• 5 years clinical RN experience
• Care management and UR experience
• ASN/ADN required
• BLS required

Preferred

• BSN or higher
• Prior care management leadership
